Homeless Prevention Program

Description:

The Homeless Prevention Program (HPP) is funded through the Emergency Solutions Grant and can provide up to 12 months of rental assistance, one-time rental appears payment, utility payment up to 12 months, and/or utility arrears to low-income households that are imminently at risk of becoming homeless and would lose their housing without this assistance.

The Homeless Prevention Program assists individuals/families that have an eviction of 14 days or less. The Success Coach and Content Specialist will work with the landlord to mediate the situation, if possible and/or assist the applicant in finding another unit.

Success Coaches will:

  1. Conduct monthly visits
  2. Focus on individuals goals
  3. Landlord/tenant issues
  4. Financial Management
  5. Other goals identified through various assessments

Eligibility Guidelines:

Once initial eligibility is determined, the applicant is placed on the Prioritization List and offered assistance when an opening becomes available in the Homeless Prevention Program in their community. Applicants are placed on the Prioritization List by their vulnerability and whether or not they meet the HUD’s definition of at-risk.
